What is Ureteroscopy?

Ureteroscopy is a minimally invasive procedure used to diagnose and treat problems in the ureter and kidney, most commonly to remove kidney or ureteral stones.It involves inserting a thin, flexible instrument called a ureteroscope through the urinary tract to access and treat stones or blockages — without making any incisions.

Types of Ureteroscopy

There are two types of ureteroscopy, chosen based on the size and location of the stone:

Simple/Basket Ureteroscopy

  • For smaller stones
  • A ureteroscope with a tiny basket at the end is used to grasp and remove the stone.

Laser (Flexible) Ureteroscopy

  • For larger or hard-to-reach stones
  • A flexible scope is used along with laser energy to break the stone into small fragments, which are then flushed out or removed.

How Is Ureteroscopy Performed?Before, During and After.

Before undergoing ureteroscopy

  • A urine test is done to check for infections.
  • You may be advised to avoid urinating shortly before the procedure.
  • Your doctor may recommend fasting and provide instructions on medications.

During ureteroscopy

  • Local anesthesia, spinal anesthesia, or general anesthesia is administered.
  • The ureteroscope is inserted through the urethra into the bladder and advanced into the ureter or kidney.
  • Depending on the site and size of the stone, the best ureteroscopic method is decided.
  • A temporary ureteral stent may be placed to help urine flow post-procedure.

After the procedure

  • You may go home the same day or stay briefly for observation.
  • Antibiotics are prescribed to prevent infection.
  • Drink plenty of fluids to help flush out stone fragments.
  • You may notice:
    • Mild pain or burning during urination
    • Small amounts of blood in the urine
    • Frequent urination — these are temporary side effects
  • Applying a warm compress near the urethra may help relieve discomfort.
  • If a stent is placed, your doctor will inform you when it needs to be removed.

Risks And Complications Associated With Ureteroscopy

Some of the most common risks associated with ureteroscopy are chances of infection, bleeding, and injury to the ureter, leading to stricture formation. Individuals who complain of blood in urine undergo ureteroscopy. It is also performed when there is an obstruction in the ureter or kidney due to stones or the growth of a tumour. Ureteroscopy is usually preferred for pregnant women and people who have stones in the ureter, very close to the bladder.

Ureteroscopy is an outpatient procedure and is a minor procedure. It takes 1-3 hours to complete the procedure.

Thedoctor will mention a few preventive measures that can be taken to reduce the chances of recurrence of stones. Some of the most common methods are taking adequate amounts of fluid, avoiding vitamin C-rich supplements, consuming more good items rich in calcium, and eating less protein-rich food.

It takes one to three hours for the procedure to complete,  provided there are not any complications.

Laser lithotripsy can cause damage to the kidneys. Using a laser to break down the larger stones into smaller fragments can obstruct the normal flow of urine. Other complications that can occur are high blood pressure or kidney failure.

It usually takes one to two days to recover from laser lithotripsy completely. The patient is advised to take a sufficient amount of water to flush out the stones. No supplement diet or additional measures are taken to recover from laser lithotripsy.

Kidney stones that are larger than 0.5 cm in size require surgery. Stones of less than 2 cm in size can be removed through ureteroscopy or shock wave lithotripsy. Stones more than 4 cm in size can be removed by percutaneous nephrostomy. Stones that are 2-4 cm in size can be removed using percutaneous nephrostomy or staged ureteroscopy.
