Dilation and Curettage (D&C) is classified broadly into two types based on the method used to remove uterine tissue. Sharp Curettage is done using a metal curette to scrape the uterine lining. The procedure for Dilation and Curettage (D&C) surgery is divided into three different phases: pre-operative phase, intra-operative phase, and post-operative phase. These phases are designed to make sure of patient safety, accuracy of procedure, and optimal recovery. The pre-operative stage is examining the patient and making them ready for surgery, which is followed by the surgical procedure itself during the intraoperative stage, and finally the post-operative stage, where the patient returns to normal activities after surgery becomes an essential objective.

Pre-operative care: Preparation before the Dilation and Curettage (D&C) procedure

The Dilation and Curettage (D&C) technique is a gynecological surgery to remove tissue from the uterine lining. It involves widening the cervix (Dilation) and scraping the uterine walls (Curettage). At the start of the Dilation and Curettage (D&C) procedure, the patient lies on the examination table, with feet in stirrups, similar to a pelvic evaluation. Then the specialist starts an intravenous line for fluids and medicines, along with monitoring the vital signs constantly.

The speculum is then inserted into the vagina to hold the vaginal walls apart and to provide a clearer view of the cervix. The cervix is then cleaned with an antiseptic solution.

A series of thin rods, with each rod slightly thicker than the last, is then inserted into the cervix to widen it gradually.

After this, a spoon-shaped instrument is then inserted into the uterus, and the lining is gently scraped to remove the tissue; a suction is also used.

The tissue that is scraped is then sent to a laboratory for analysis of the underlying condition. Once the procedure is done, the instruments are removed, and then the patient is kept under observation.

Post-operative care: Dilation and Curettage (D&C) procedure

Dilation and Curettage can be performed for various reasons, namely, in diagnosis, they are used to check the cause of heavy uterine bleeding, to check for the growth of uterine cancer, or abnormal polyp growth, and to check for infertility. In treatment, it is used to clear the uterine tissue after a miscarriage or abortion, to remove the extra placental tissue after the birth of a child, to treat uterine fibroids or polyps, and in case of an elective abortion.

Dilation and Curettage is an outpatient procedure, carried out under general, regional, or local anesthesia. During the procedure, the patient will be positioned on the evaluation table with the feet in stirrups. A speculum is then inserted to hold the vaginal walls open so the doctor can see the cervix. The cervix is then gently widened using a series of thick rods known as the dilatators. After dilation, a curette is then used to scrape or suction the tissue from the lining of the uterus. This tissue is then sent to the lab for analysis.

The recovery from Dilation and Curettage is quick, and the patient usually recovers and returns to normal activities within 1 or 2 days. The patient may experience mild cramping and vaginal bleeding for a week or two, and should avoid heavy lifting and sexual intercourse. Tampons should also not be used to prevent the risk of infection. Emotional recovery is also important, especially after the loss of a pregnancy. The risks associated with Dilation and Curettage are heavy bleeding, infection, and perforation of the uterus.
