Expert Anterior Cruciate ligament (ACL) Treatments at Yashoda Hospitals

Athletes and women frequently wearing high heels are at a higher risk of experiencing a ligament tear. The Anterior Cruciate Ligament (ACL) is crucial in the knee, connecting the tibia (shin bone) and the femur (thigh bone). When this ligament is torn or sprained, it results in an ACL injury.

Anterior Cruciate Ligament (ACL) reconstruction is a surgical procedure designed to restore knee stability and function by repairing or reconstructing the damaged ACL. During the procedure, a tissue graft replaces the torn ligament, often using a tendon from another part of the knee. This minimally invasive surgery involves small incisions around the knee joint and is typically performed as an outpatient procedure.

At Yashoda Hospitals in Hyderabad, our highly skilled ACL surgeons have extensive experience in successfully treating ACL injuries, ensuring patients receive top-tier care and a smooth recovery.

Who Needs ACL Surgery?

  • Individuals with frequent knee instability
  • Athletes seeking to return to high-demand sports
  • People with physically demanding jobs who wish to seek an active lifestyle
  • Individuals with associated knee injuries like torn meniscus
  • If symptoms persist despite physical therapy and other treatments

Types of ACL Injuries:

Approximately 50% of all anterior cruciate ligament injuries coexist with harm to the knee’s meniscus, articular cartilage, or other ligaments.

  • Grade One: In a Grade 1 sprain, the ACL is slightly stretched but not torn, representing mild damage to the ligament fibres. Therefore, the knee’s stability is still supported.
  • Grade two: A Grade 2 sprain stretches the ligament until it becomes loose, a condition commonly referred to as a partial ligament tear. The knee’s stability is slightly disturbed, and associated symptoms include pain and swelling.
  • Grade Three: A Grade 3 sprain completely tears a ligament (either into two pieces or completely away from the bone), severely destabilising the knee and making walking difficult. This injury is referred to as a complete ligament tear.

What to Do Before the ACL Repair Surgery?

  • Engage in physical therapy to reduce pain and swelling while simultaneously working to maintain lower body strength.
  • Exercise regularly to improve balance and gain proprioception, mainly focusing on quadricep muscles.
  • Inform the surgeon about any ongoing prescription, supplement intake, and any active allergies.
  • The surgeon may advise against certain medications, especially blood thinners and instruct their patients to fast a few hours before the surgery.
  • Restrain from nicotine, caffeine, and alcohol consumption.
  • If needed, practice with crutches for post-surgical convenience.

What Happens After the ACL Repair Surgery?

  • The patient may experience immediate pain and swelling, which painkillers can manage.
  • Follow the wound care instructions given by the surgeon, which may involve keeping the wound clean and dry and changing the dressing.
  • Rehabilitation therapies such as focusing on knee extension, gentle leg raises and ankle pumps, and gradually increasing the weight on the treated leg play a vital role in the healing journey

Types of ACL Surgeries:

There are two main types of procedures for ACL injuries, i.e., ACL reconstruction and ACL repair.

ACL Reconstruction (single bundle):

Single Bundle ACL Reconstruction Surgery involves the removal of the damaged ligament from the knee and inserting graft tissue (autograft/allograft), taken from tendons in and around the knee, to restore its function. Arthroscopic ACL reconstruction, which only requires a few little incisions around the knee, is a less invasive option than open knee surgery. During this procedure, to obtain a clear image of the torn ACL, your doctor will insert a camera and other tiny tools. The graft will then be held in place by the two tunnels they draw. To prevent harm to your joint, you will need to wear the brace.

Autograft ACL Reconstruction involves the creation of new ligaments from the patient’s body tissue, such as patellar tendon autografts, hamstring tendon autografts (tendons from the back of the thigh), and the quadriceps tendon above the kneecap. In contrast to this, in allograft ACL surgery, the tissue is taken from a cadaver.

Compared to its traditional equivalent, arthroscopic ACL restoration is safer and superior since it involves fewer incisions and causes less tissue damage. It also recovers fairly quickly.

Double-Bundle ACL: The anterior cruciate ligament (ACL) is surgically replaced with two smaller grafts as opposed to one larger graft in a double-bundle restoration. In this procedure, the graft material is passed through three tiny “portals” (5 mm incisions) and a tiny incision (1/2 inch) in the shin bone.

Primary ACL Repair: Primary ACL repair is considered one of the surgical procedures for ACL tears. During this procedure, donor grafts are not being used, and the ACL’s torn ends are cautiously aligned, sutured together, and trimmed to eliminate any frayed or damaged tissue.

FAQ's

The ACL reconstruction surgery cost in Bangalore can range significantly from approximately Rs. 70,000 to Rs. 2,20,000, depending on several factors, including the specific surgical technique, the type of graft used, the length of the hospital stay, and the surgeon's experience.

While both ACL repair and reconstruction are surgical procedures used to treat ACL tears, they take different approaches to the injured ligament. ACL repair includes mending the existing damaged ligament, whereas ACL reconstruction is replacing the damaged ligament with a graft.

The main signs your ACL Repair is healing well enough include reduced knee pain and swelling, no significant stiffness in the knee, allowing normal movements, and meeting the rehabilitation milestones such as Walking without crutches, etc.

Without surgery, an ACL tear—especially a complete tear—cannot heal naturally, as individuals with ACL injuries lack proper blood flow. Non-surgical therapies such as physical therapy and bracing may help in cases of partial tears; however, individuals with complete tears surely need surgery for complete recovery and stability.

It often takes nine months or more to fully recover from surgery, including physical therapy and the initial phases of recovery. To avoid re-injury, it’s crucial to wait until your ACL restoration has fully healed before getting back into sports.

While ACL surgical reconstruction is generally recommended for younger patients who are more active, PRP injections may be better for older patients for improving knee health. Nevertheless, these injections also help individuals with post-ACL reconstruction by enhancing the recovery process.

Rest, ce, and gentle exercises are the first steps in the Acl Protocol with a hamstring graft. Over several months, strength training and functional activities are added. The complete recovery time span may range from 6 months to 12 months.

Like any other surgery, knee surgery involves a few minor risks, such as infection, blood clots, knee discomfort, and stiffness

You may begin with a few exercises, such as quad sets, hip flexion exercises, and heel slides, following an ACL injury. Other exercises include hip abduction exercises, glute sets, bridging, shallow standing knee bends, and heel lifts, which can be performed as your symptoms subside and in case you can support your weight.
