SECTION B — MODE OF PARTICIPATION (tick one)

In-Person — Yashoda Hospital Auditorium, Somajiguda, Hyderabad

Online — live video session

SECTION C — ONLINE PARTICIPANTS ONLY

I confirm I will join from a single device, with my camera switched ON and my face clearly visible for the full duration of the lesson (minimum 30 minutes). I understand that participation without a visible video feed cannot be counted towards the record.

SECTION D — CONSENT DECLARATION (mandatory for all participants)

I confirm I am 18 years of age or older. I consent to the collection of my registration details (name, contact information, and attendance record) by the IBD Conclave 2.0 organising team, and to their sharing with Guinness World Records Ltd solely for verification of the record attempt “Most people in an inflammatory bowel disease awareness lesson online & single venue” on 10 July 2026. I understand that photographs, video recordings, and attendance logs from the event will be used as evidence for record adjudication and retained per GWR’s evidence requirements. I further consent to the use of my registration details to issue my certificate of participation. My personal data will be processed in accordance with the Digital Personal Data Protection Act, 2023 (India) and applicable UK data protection law, and will not be used for any other purpose without my consent.
